2024.11.29 02:00 Apart_Ambition_2293 Is this normal? (OTA FW1 student observing in a SNF)
I’m an OTA student in fieldwork 1 observing at a long term care facility/SNF. All patients at this facility receiving OT are billed for 45-68 minutes, depending on whether they have Medicare Part A or Medicare Part B. The time starts the minute you walk into the room and includes time spent documenting.
One of the OTAs I was observing was treating a patient with contractures on both sides of her upper and lower extremities. The patient screamed nonstop before, during, and after we left her room. We repositioned the patient, used a carrot to open her hand, applied hand/wrist splints, and walked out within 15 minutes.
As we were walking out, the treating OTA said in a frustrated tone that there was nothing else we could do for the patient. When I asked her how something like this would be billed, she said it would be billed for ~55 minutes. Confused, I asked how that was possible. The OTA proceeded to explain to me that this was normal practice in SNFs, and implied that therapists are forced to do unspoken, unethical things in order to meet productivity demands (which is 90% or 93% for OTAs at this facility). She also said that rehab directors don’t care how you make it happen.
I asked the treating OTA if it would be reasonable to recommend the patient for discharge given the circumstances, and she said technically yes, but that the patient would just end up getting picked back up by the rehab department for contracture care.
My OTA clinical instructor who was present, added that technically the treating OTA could document that the treatment was ~55 minutes because she would have to return to the room to remove the splints within the hour.
Is this normal practice in SNFs? As an OTA or OT would you do something different? What are your thoughts?

im in an emo band and regularly use vocal fry and false chord tehcniques for screaming and yelling. normally i have no issues and i'm able to push myself on highs and lows, but recently i've kind had to find a sweet spot and if i don't i end up coughing a bunch or just get general discomfort which i've never had before. is there something i can do to get my vocals back to normal? maybe a warm up? or just anything i can do to help recover and prevent myself from straining?

14M slightly overweight.
I don’t think it’s a problem with breathing itself but sometimes I just forget to breathe for like 10-30seconds or I breath once every 5 seconds for a few minutes and then my chest hurts for a solid few minutes. Honestly I don’t think it’s an issue I’m just curious if this is unique or not. I don’t have any other breathing issues, it’s just psychological I think
