My husband has never had any training on how to bill a G0402 (or 0438/0439) with an E/M visit. We've been operating a small family medicine clinic for over 2 years and our billing is outsourced locally. Our bill has never mentioned to us that this is something we could be bundling.
We've been really looking into it because he has lots of medicare patients. He did his first (combined) encounter yesterday (billed both the 99214 and the G0402) and I just saw that our biller sent it off and deleted the 99214 from the claim (no call, email, no communication). We have struggled getting good information from them in terms of what can be billed and what can not.
I know that they can be billed together as long as the documentation supports necessity for both but he felt he did that and is unsure of how to document it better to demonstrate this. If I post his note (no personal information of course)...would anyone be willing or able to give us some feedback on how it could better demonstrate the medical necessity for billing both?

appearance changes in 6 months post TT. I recently posted how I feel post TT and thought it would be useful to show how my appearance has changed over the last 6 months. I’ve gained 18 pounds of what I’d consider healthy weight. The bags under my eyes are gone. My face has filled out and I look much healthier than before. Im 6’2 and was 190 before surgery. I was lean and was hard to build muscle. Now I’m 208 and I can see the all the hard work pay off at the gym. Hope this helpful for someone.

Hallo Zusammen, wer kennt sich mit der Sicherheitsarchitektur der "epa für alle" aus, die demnächst kommen soll?
Ich habe dazu die Diskussionen der letzten Wochen verfolgt und mir die Slides vom ccc angesehen. Der ccc schreibt "Mängel in der Spezifikation es ermöglichen, Zugriffstoken für Akten beliebiger Versicherter zu erstellen". Die Daten in der Akte sind angeblich verschlüsselt.
Dabei ist eine primäre Frage für mich mit welchen Schlüsseln das erfolgt. Offensichtlich ist es kein Schlüssel, der allein in der Hand des Patienten ist. Sonst könnte nicht irgendeine Stelle (welche ist das?) einen Token erstellen, um auf Akten beliebiger Nutzer zuzugreifen und sie zu lesen.
