Hi all. This will be lengthy but I'm desperate for help and appreciate any insight. I'm a nurse and help care for my mom who is ill, but getting answers or help has been so hard and is now very convoluted. This is her story.
Demographics: 69 years old, white female, 5'5, about 170lbs.
Clinical history: Noted, significant hypertension around 2019/2020 but as she had severe anxiety with doctor appointments it wasn't followed up on. No other significant medical issues.
Family history: Father died of heart attack aged 54, mother died at 94 of GI bleed. Sister died aged ~75 of stroke. No known family history of dementia. Daughter with celiac disease and bipolar disorder (me.) Son has IGA deficiency. No other significant medical history in her children.
Onset of symptoms: Very gradual short-term memory loss starting around 2019. This is an estimate, however we really didn't notice any significant problems until about 2021 when she became more forgetful. It progressed from there.
March 2022: Her primary care provider diagnosed her with Alzheimer's disease. Referred to neurologist who seemed to agree with diagnosis. MRI was done, states "extensive white matter disease, severe microvascular changes" and not to rule out multiple sclerosis. None of what was in the MRI report was even mentioned to us by her neurologist. Diagnosis remained AD and she was started on Galantamine and Memantine, along with hypertension medication.
(EEG also done but was unremarkable.)
Symptoms progressed to severe short-term memory loss, change in personality, deteriorating mobility (shuffling gait.)
Second neurologist as of 2023-2024: Same meds continued. Memory tests conducted every 3 months, she obviously failed them. We were under impression she was still being tx'd for AD. Suddenly neurologist says "oh this is amnesia, she could get better." WTF.
We requested medication to ease worsening symptoms of SEVERE agitation, anxiety, occasional aggression. Were told to take her to psychiatrist.
So we did that. Notes say possible ichemia. No confirmation or denial of if it's actually AD or another form of dementia. She was prescribed an anti-psychotic which exacerbated her agitation ten-fold. Cannot even get a response from the office regarding this yet. She has a follow up appointment in a month.
So, here's the status quo as far as her symptoms:
-SEVERE short-term memory loss (almost zero recall). Long term memory intact (remembers childhood etc). Still recognizes family. -HALLUCINATIONS almost all the time, usually vivid and of animals or people, dead and alive. Very upsetting and disturbing. -Severe sleep disturbances. Hypersomnia during the day, insomnia at night. -Severe, constant delusions, about pretty much everything. Imagines all sorts of scenarios and conversations that did not happen. -Mobility has deteriorated. She shuffles and barely lifts her feet when she walks. As a nurse when I see her gait I do not think she will be ambulatory much longer. -Severe agitation and anxiety, almost all the time. Occasional aggressive outbursts when frustrated. -Severe, transient episodes of nausea, diarrhea, and almost vomiting. They seem to be cyclic attacks occurring randomly a couple times to several times a week. -Total incontinence, urine and fecal. -Severe bilateral foot pain and swelling. -Deteriorating ability to perform ANY ADLs (hygiene, dressing, bathing etc)
Additional notes: She's under the care of a urogynecologist and does not have a UTI. Her lab work is always normal. Her hypertension is well controlled and has been for 2 years.
I'm completely at a dead end at this point. Everywhere we go they don't communicate with us or clarify what her actual diagnosis is or anything, it's so confusing. I don't even know what her baseline is anymore because of conflicting information and the meds she's on. All I know is she is miserable.
Does anyone have any insight? Does this actually sound like textbook dementia? Could it possibly be something else? I read about AD potentially being considered an autoimmune disease now? Why won't any of these doctors do ANY other tests? We can't even get one who will treat her symptoms such as agitation or anxiety.
I worked with Alzheimer's patients for years so of course a lot of these symptoms align, but none of my patients were violently ill all the time so if that is unrelated...wtf?

I would love the AMD option due to more vram and overall price to performance balance but I've heard they are not compatible with some programs or are significantly slower than the nvidia ones. am I doomed to buy an rtx?

My partner and I are about to go on our first international trip. It was planned a year in advance and it is a family trip, not a romantic get away. Lately we are not communicating well.... like arguing over stupid things, spiraling when one of us is stressed, trying to please the other person and neglecting ourselves to the point of resentment.... yeah.
We have been married for 10 years. We are both in therapy separately and tonight clearly decided we need to seek some relationship counseling because we love each other and want to make it work, but also continually make each other feel like shit when we try to communicate through anything emotional. This has been a problem since our youngest was born and she's PreK aged now.
We are about to be stuck on a flight together with our two young children for 22 hours (not including layover time) and then 14 days of family holiday time.
Any tips on how to survive the trip without biting each other's heads off in the airport? We want to make this a pleasant family trip for our children and other relatives.... ideally, this trip would bring us back together and we woukd be closer at the end but I'll take neutral at this point.
TLDR seeking tips on a long family holiday with a partner who is actively seeking marriage counseling while I type this. Trip in 20 days.
