I have a young schizophrenia patient with breakthrough psychosis on Abilify 400 mg IM q4wk depot. I’ve switched to Invega by giving Invega 100 mg q4wk + 6 mg PO on the date that Aripiprazole was next due (without giving Abilify itself). I was recommended not to load Invega to avoid adverse effects.
Within 2-3 days of starting Invega, psychosis seems to have stabilized, but I’m noticing more negative symptoms. I’m assuming secondary negativism from antipsychotics - maybe I’m wrong?
I’m struggling to understand the mechanism of these side-effects when switching from a D2 partial agonist (Abilify) to D2 full antagonist (Invega)? On Puder’s psychotherapy podcast, Dr Cummings says that assuming a partial agonist is occupying more than 80% of the D2 receptors, my antagonist is not going to have much of a target. He says it’s okay to initiate an antagonist, stop the partial agonist, and it will then gradually wash out over a long time. I think this is what I've done.
Since Abilify is working as an agonist or antagonist based on local dopamine concentrations, how is it possible that Invega can cause side effects if Abilify is occupying D2 receptors?
Where is Invega going to potentially cause secondary negative Sx if Abilify is theoretically already occupying those receptors?
Is Invega acting somewhere else where Abilify has no effect?
OR is it maybe just that Abilify is actually not even close to 80% occupancy, so Invega is causing excess dopamine blockage in all regions, leading to improved positive symptoms but also negativism??
I feel like I’m likely misunderstanding the mechanisms here. Any help is appreciated!!

Recently, I’ve seen many videos of people physically assaulting taxi drivers. In one, a woman was hitting a driver with her bag, and in another, someone grabbed the driver’s shirt and pushed him against a wall.
I’m not justifying physical violence or being extremely rude to them, but there are always two sides to every story, and we usually only see one.
Some taxi drivers here can be rude, take Salik unnecessarily, or choose longer routes to increase the fare. They don’t always maintain hygiene, often wearing the same clothes, smelling unpleasant, or even passing gas in the car.
If you’re going through something stressful, they sometimes initiate forced conversations, ask personal questions, or start complaining about their struggles, like low salaries or family responsibilities. Some even criticize the RTA. When you reach your destination and pay the fare—let’s say it’s AED 50—they might act disappointed or rude because they expect a tip after sharing their sad stories. Sometimes, if you pay in cash, they throw it back in frustration if it’s not extra.
They often come across as intimidating and use their physical presence to make it clear there will be consequences if you don’t pay. That’s why I prefer to pay them in advance to avoid these situations.
In my opinion, no one has the right to assault workers, but taxi drivers should also be respectful and professional. To address this, I suggest installing cameras in taxis to record everything and implementing a system where fares are paid in advance.

 | Painting of my first models. The very first was the one with the more brown skin tone. Had some paint get rubbed off when doing the streaking grime step. The rest were painted assembly line, tested a few different priming methods to see how they affected painting. Though, the biggest problem was brush control and keeping the paints confined to their specific spots (which didnt work as well as i wanted).
