2024.11.25 16:57 Traditional-Care-87 [Hypothesis] Nutritional requirements for maintaining Cymbalta
I have a bizarre (silly?) hypothesis about SSRIs and SNRIs, and I'd like to hear your opinions on it.
I was using Cymbalta, but the effect disappeared after two months, and even increasing the dose no longer worked at all. (It had worked perfectly until then.)
This is called the poop out phenomenon, and someone on reddit told me that it is also related to receptor downregulation.
I accepted that, and while searching for other ways, I came across Piracetam.
When I took Piracetam, my brain fog and fatigue were significantly reduced. However, I heard that Piracetam sometimes requires choline, and that if you do not take that nutrient, the effect will disappear or a headache will occur.
Here, I felt that Cymbalta and Piracetam might have a similar mechanism.
In other words, just as Piracetam requires choline (some people need it), the hypothesis came to me that Cymbalta also requires some kind of nutrition to maintain its effect, and if you neglect to take it, it will no longer work.
Of course, the mechanism by which antidepressants stop working may be different for each person, and I do not claim that my hypothesis completely explains everything.
However, to simplify things, I thought that for example, in the case of Cymbalta, the effect may be maintained if you take something that is a source of nutrition for serotonin and norepinephrine.
The questions I would like to ask here are:
① Like the relationship between piracetam and choline, how likely is it that a deficiency of a specific nutrient is involved in the weakening of the effectiveness of Cymbalta (or SSRIs and SNRIs in general)?
② Based on my hypothesis, what nutrients do you think are important for making Cymbalta work again?
③ I thought about taking B vitamins and magnesium for serotonin, and copper and vitamin C for norepinephrine, but is this a hasty idea?
④ This may be off topic, but I have a strange reaction to vitamins. For example, when I take vitamin B (especially vitamin B12), I get auditory hallucinations and feel fatigued. Is there something wrong with methylation? In this case, should I selectively take only the B vitamins involved in serotonin synthesis?
⑤My main symptoms are brain fog, ADHD, chronic fatigue, dry throat, and dry eyes. Why do psychiatric drugs solve these? Cymbalta improved these "physical and physical" symptoms all at once in the first two months. I have almost no psychiatric symptoms at all.
This is a long story, so a partial answer is fine. My life was a mess because of chronic fatigue and brain fog, and I really felt like I got my life back during the period when Cymbalta was effective (not manic), so I hope Cymbalta will work again. Is there any good way? I'm ignorant, so please feel free to point out any points that may be useful, even if they are unrelated to my hypothesis.

Seeing a movie in an almost empty theater is one of my favorite activities, but I've heard a few variations of "the studio makes more money the first couple weeks."
I don't care about making money for studios, per se, but I want to show support for smaller or independent films. If it's already a huge blockbuster then who cares, but if it's a limited release then I want to signal to studios that these are good movies to invest in.
Does it make a difference if I see it in the first week, vs the third or fourth? I'd love to avoid crowds (and all the idiots on their phones) but I don't want to lose independent movies in mainstream theaters.

I'l try to keep this post relatively short. Cause i'm a person who is very good in making very long stories.
Im considering accutane and i got permission from my dermatologist to start. One blood test and i could swallow the first pill as soon as 1 week. See the image attached to see my skin status right now.

Im not sure to do Accutane cause:
- Im scared of side effects: Tinnitus (ringing in ears), blurry vision, hairloss, permanent dry lips, skin, eyelids, etc. Joint pain, Headaches and what not. Basically any side effect that will continue after the accutane cure. I dont want it and thats why im considering doing a very low dose accutane or even using it every other day so i try to avoid side effects as much as possible.
- I dont drink much water - I never reach 2 litres or 33oz in a day, i maybe drink 2 or 3 big glasses at most and that's it. Will Accutane destroy me if i keep up this habit of dr1nking so few? Or should i REALLY improve my hydrating before even starting? (Even if the liver test shows my liver is healthy)
- Fear of it coming back: This explains itself. Relapse rate (Rate of how often acne comes back) is about 35%. So i feel like Accutane is a 1 in 3 gamble that the acne will come back, or pretty much stay away forever. Im too scared to risk it (I think)
- I feel like i should try all the other options: As this title says, i feel like i should try all the creams, serums, diets out there before Accutane. Personally i think Accutane is a very extreme option for how bad my skin is (see image) And thats why im desperate to try all the products out there before Accutane as last resort.
What do you think when you read this? please let me know.
- Do you think i should stop whining, start already and take the risk? - Or do you agree for example that i should try a diet, different face wash, and live more healthy.

Hello to everyone. Witch as a class got a lot of attention from Paizo since remaster — familiar abilities, subclasses, feats. Some of this content is good and flavorful, some of it is… questionable (why do they keep insisting on Witch's Armament playstyle, it's not going to happen, Witch has a durability of a wet potato chip.) War of Immortals and Divine Mysteries had a lot of options for the class, so I wanted to share my thoughts about them.
Little heads-up: some of my analysis will be a bit pedantic and probably will have "you must be fun at parties" vibe. That's because I think that PF2e is a very literal game and intend to view it as such.
Let's get started.
Seneschal Class Archetype What a great archetype. Love the flavor, love the option to play as a witch that is no longer bound by the patron’s desires, as per description. However, mechanically, it has some erratable issues.
Manifest Will and Dedication Feat
Seneschal takes away your normal Hex cantrip and replaces it with Manifest Will. Hex cantrips are important for the remastered Witches as they enable constant use of your unique familiar ability, so naturally Manifest Will is one of the central parts of Seneschal playstyle.
Here's where problems are starting to show up:
You are concealed from creatures beyond the edge of the emanation but can’t use that concealment to Hide.Okay, sounds very simple: runes\symbols\energy\plants are swirling around you, making you concealed. But here's the thing: Manifest Will is one of the only ways to center an emanation on someone else (as per dedication feat at level 2).
When you cast or Sustain manifest will, as long as your charge is within 30 feet, you can choose to center the spell’s emanation on your charge instead of yourself.So, what about concealment? Everyone, except from creatures right in front of your charge, are "beyond the edge of the emanation". Should you be concealed from everyone? Is that the intent? I hardly believe it. Here's possible interpretations of what does it do when you're centering Manifest Will around someone else and you yourself isn't in emanation:
- The most literal and RAW interpretation is that you are, well, concealed from everyone beyond the edge, ergo you are concealed from almost everyone. This has absolutely zero narrative meaning as there are no flying symbols obscuring vision, and it's overly powerful. I don't think this is in any way intended.
- Since you are outside of emanation, "beyond the edge" works the other way around and you are concealed from the creatures that are in the emanation — because that's the "edge" from your perspective. This interpretation has more merit as narratively it kinda works: symbols are obscuring vision of creatures inside.
- Since now symbols are swirling around your buddy, they are concealed, and not you. Narratively it works perfectly. The spell, however, clearly says "you". Also, it would make the core features of the archetype work really awkward with each other: if your charge is concealed from you, you have a chance to fail when you try to cast something on them. The whole idea of Witch's Charge is to be able to cast spells on the charge, and it has a really bad anti-synergy with concealment.
- Since you are not in the emanation, it has no effect on you. The only way to get concealment is to be close to your charge.
Multifaceted Will
If you thought I'm done with Manifest Will, I'm sorry. Multifaceted Will, 6th level feat, says:
When you cast or Sustain manifest will, you can choose any tradition’s manifestation.It's awkward beyond belief. Say, you cast Divine Manifest Will, including a creature in the center square of an emanation in it (yourself or charge, doesn't really matter). Next turn, you sustain and want to change to a Primal or Arcane. Well, emanation already incudes a center square, so you\charge will get a penalty to speeds\weakness to spells. It works the other way around too. If you cast Arcane\Primal and then sustain and change to Divine\Occult, well, you probably didn't include a center square.
Spiritual Secret (link)
Very powerful spellshape for blaster Witches. The notable thing here is the fact that is affects all the damage spell deals — not only an initial damage and also works on spells with duration. It makes the spell sanctified, so taking an archetype with sanctification can be very handy. If you are Holy, you can deal persistent Holy damage instead of usual bleed\fire\acid — it's bound to melt fiends with weakness to it.
The last thing with this spellshape is that if a spell deals mental damage, you can remove mental trait with this and target mindless creatures with all effects of the spell, not just damage. Just remember that emotion trait, should the spell have one, still will make them immune.
How Seneschal interacts with different patrons?
Well, Seneschal takes away a hex cantrip, but leaves a familiar abilities as is. That means that your perfect seneschal patron should have a really good ability for familiar.
For example, Ripple in the Deep has a very good (probably overpowered, if I can be honest) familiar ability, but its hex is meh. Not bad, but definitely not exciting. Manifest Will will replace it, but still will leave your really good ability. At the same time, Wilding Steward has a really good hex, but probably the worst familiar ability. Seneschal doesn't seem to be a good fit, since you will lose the best part of your subclass.
Faith’s Flamekeeper can give temporary HP, but divine Manifest Will already does that. Workable, but awkward.
Choir Politic can get a combat-oriented hex cantrip if they aren't a fan of having more exploration-focused one to trigger its Bolstering Aid more often.
Patrons from Divine Mysteries Despite the name, only 2\5 are actually Divine.
Choir Politic
It has a very unique hex cantrip — it's not a spammable tool for combat like other patrons ones. Bolstering Aid too is a rather narrow tool in combat, but a very useful one for exploration.
Fan Fact: Patron skill of Choir Politic is society. Not Religion. Your patron did not teach you a knowledge of Divine magic, but rather a genealogy and history. You can play as a spellcaster that's not trained in their tradition's skill.
I don't know if Share Vision should work with abilities like Cloud gazer. Narratively, it makes so much sense, but I'm not sure if it was intended to work together. I think, most DMs would agree, so here's a little list of things you could combine with Share Vision: Cloud gazer (+ Fog spell), Firesight, Fire Lung, Winter Cat Senses, Goz Mask, Wortwitch, Murksight, Emberkin. u/leathrow made a great post about it, so check that out if you are interested in fun applications of this hex.
Paradox of Opposites
A great patron, I don't really have much to say, except for one thing: it's familiar ability can greatly simplify certain fights, and between Disrupt Opposed Magic, this and Exemplar's Victor’s Wreath there is a lot of effects that has the potential to make some fights trivial lately. Not a fan of this approach to design, personally.
The Unseen Broker
Aesthetics are great. Mechanics… can be lacking. Pact Broker has Linguistic, Mental and Auditory traits, and it limits its usefulness greatly. As if it wasn't enough, it also makes targets immune for one minute. The effect is good, but is it good enough to justify such harsh limitations? I don't think so.
And also I'm disappointed by the decision to not remaster Lesson of Favors and Lesson of Bargains, considering that they remastered subclass, tied thematically to these Lessons.
Baba Yaga
Spirit Object has no crit effect, only normal success. It's also a spell attack roll (good thing there's Sure Strike in occult) and as such has an accuracy of a blind newborn zebra foal. Not to mention the fact that you can't move it and attack in the same turn, so you have to hope that there will be an opponent in the space you moved your object into. The damage is good (again, no crits).
You can manipulate objects on the battlefield with things like your familiar with manual dexterity, telekinetic hand and telekinetic projectile, but that sounds a bit gimmicky. You can also interact to draw a potion, drop it and cast Spirit object on it, so it would walk to your ally. Again, gimmicky, but fun and cute.
On higher levels it can allow you to move objects of higher Bulk — it can be useful in some scenarios.
Familiar of Obscuring Snowfall is a bit awkward too. Enemies have to actively move to enter the space and trigger dazzle, but it still seems pretty decent.
Despite flaws and all the awkwardness, I really like this patron.
Couple of other things Well, a lot of Witches has access to a repeatable source of concealment and dazzle (which is the same thing). Because of this, Mist Child (changeling feat) can be very useful and protect you from a hit or two during some battles.
Also, blaster witches may appreciate new Emissary Familiar feat from Rivethun Emissary Dedication.
Conclusion First of all, if you read all of this, huge thank you. I really like witches and most of the new content for them, but I tried to be objective and non-biased during analysis. Feel free to express your opinion,
